Superbills

A superbill is an itemized receipt for a self-pay visit. It lists the date of service, the diagnosis and procedure codes, the provider’s NPI, and the amount paid, in the format insurance plans use to review out-of-network claims.

We provide a superbill after any self-pay visit on request. You submit it to your plan; whether and how much your plan reimburses depends on your out-of-network benefits and deductible. Payment is due at the time of the visit regardless of reimbursement.

Superbills are available for clinical visits and pelvic floor therapy. HSA and FSA cards may be accepted.

Do you take insurance?

What does a visit cost without insurance?

$250 for a new patient and $150 for a follow-up. Pelvic floor therapy is priced separately, at $210 for a new patient and $180 for a follow-up.

Are labs and prescriptions included?

No. Where your insurance covers them they are billed to your plan, and what you pay depends on your individual insurance plan coverage.

Can I use my HSA or FSA? Can I get a superbill?

HSA and FSA cards are accepted. A superbill is provided after any self-pay visit on request, for you to submit to your plan for out-of-network reimbursement. See Superbills above.

Is pelvic floor therapy covered by insurance?

Pelvic floor therapy is self-pay for now, and we are working to add insurance for it. We provide superbills you can submit to your plan, and we accept HSA and FSA.
